The crown is not the hairline
The crown, or vertex, is the highest point on the back of the head, and it behaves differently from the front. The skin there is thicker and the blood supply is more limited, which makes each graft harder to feed in its first days. The hair also grows in a circle - the whorl - rather than forward, so every follicle has to be placed at its own angle and depth. Getting that wrong does not look like thin hair; it looks wrong, and it is difficult to correct later. The crown hair transplant page explains how the whorl is planned.
Hair loss that is still moving
The commonest reason a crown result disappoints has nothing to do with the surgery. If the loss around the crown is still progressing, the transplanted hair stays while the native hair around it keeps falling, and what is left is a transplanted patch ringed by bare scalp - the donut effect. It is why a surgeon may advise treating the loss medically first, or transplanting the front and waiting on the crown, rather than filling the vertex at twenty-five.
The crown takes more grafts than people expect
The vertex is a broad, curved surface, and covering it convincingly costs grafts. By Norwood stage, a man at stage 4 with front and crown loss is usually looking at 2,500 to 3,500 grafts, stage 5 at 3,500 to 4,500, and the advanced stages beyond that. The donor area is finite
Every graft placed in the crown is a graft no longer available for the hairline, and the donor area does not grow back. For advanced loss, transplanting 6,000 or more grafts in one day is not safe - it can traumatise the scalp, cause shock loss and lower the survival rate of the grafts - so the work is split into two sessions, and beard grafts are sometimes used to extend what the donor area can give. A plan that spends the whole donor area on the crown in one go is a plan to regret.
What a good crown plan looks like
It states how many grafts go where, it says whether the hairline or the crown comes first, it is honest about whether a second session will be needed, and it accounts for the hair you may still lose. Techniques matter less than that: FUE, DHI and Sapphire all place grafts in the crown, and the choice follows the plan rather than the other way round.
Questions worth asking before you book
How many grafts for the crown, and how many for the front? Is my loss stable, and how do you know? What happens to the hair around the transplanted area in five years? Will one session be enough, and if not, when is the second? Who places the grafts? Can the crown be transplanted twice?
Yes, and for advanced loss it is often planned that way from the start, because taking every graft in one day risks the scalp and the survival of the grafts. The second session waits until the first has grown out.
Why does my crown look thin after a transplant?
Either the area was covered with fewer grafts than it needed, or the native hair around it has kept falling since. Both are visible in a photograph taken a year apart, which is what a surgeon will ask for.
Is the crown worth transplanting at all?
It depends on the donor area and on whether the loss is stable. Where the donor is limited, spending it on the hairline gives a change that frames the face; the crown is only seen from behind and above.
